AFAIK, XIV had a 2 instance limit since launch, and you needed minion to launch more than 2. Maybe that changed/got dropped at some point, but even as recently as the free login campaign, I could not log into more that 2 without minion.

Attach to one or more games before launching #3 and it should work.
