I've been using this profile for months, ever since it was released but with the new changes to skill manager a few things need to be changed in order for it to work properly. Apologies to the original creator if I'm stepping on toes here.

Summon
You need to remove 150 from PREVIOUS SKILL ID in order for this to summon pets correctly with swiftcast.

Aerial Slash
Change SKILL TARGET to "Cast Target".

Contagion
Change SKILL TARGET to "Cast Target".

Shadowflare
You need to remove 150 from PREVIOUS SKILL ID in order for this to cast Shadowflare correctly with swiftcast.
