Yes it happened to us late at night. Someone left and the leader forgot to stop the duty and it entered us (insta queue) with a random dps. Luckily we stopped it upon entry.
You had a tank and healer? That seems strange. One of the biggest reasons it doesn't happen is that generally you use 4 dps when doing teleport farming which means that if one leaves you can't queue as a 3-man due to role requirements.

yes this is correct this is why you should always use 4 dps.
